The title of the story comes from the Bible and depicts the Virgin Mary in grief as she holds the crucified Christ in her arms. Christ lies between the Virgin's knees, a scar on his ribs, his head hanging back, his right arm resting on the Virgin's right knee, and the Virgin's face appearing young, dressed in a robe and cloak, her left arm stretched back and her right hand holding Christ.

Michelangelo, in a departure from tradition, portrayed the Virgin as a young girl, saying, “The Virgin Mary is the embodiment of purity, sublimity, and the symbol of divine things, and so is bound to be able to retain her youthful and beautiful countenance for all eternity. Her image of eternal youth and nobility symbolizes the ideal of mankind's pursuit of beautiful things.” The composition is pyramidal, with the robes accentuating the silhouette and being able to reconcile the conflict between the beauty of the composition and the actual proportions. The surface of the statue is meticulously polished, smooth and shiny. The Virgin's chest sash is signed by Michelangelo, his only signed work.
